Disputed Questions of Prayer: The Four Schools in Their Own Books
Four frequently asked questions about the prayer: reciting al-Fatihah behind the imam, pronouncing the basmalah aloud, the qunut of Fajr, and raising the hands when bowing and rising from bowing — presenting each school's position from its own authoritative books, with its evidence quoted verbatim and its source named, without preferring one position over another. Considered scholarly disagreement is a breadth, not a defect; the aim is that a Muslim knows the basis of the position he or she follows, and where it comes from.
Must the follower recite al-Fatihah behind the imam?

Hanafi school — The follower does not recite at all; his reciting is prohibitively disliked
For the Hanafis, the follower does not recite behind the imam at all — not even al-Fatihah in the quiet prayers, by agreement of their three imams; if he does recite, it is prohibitively disliked (karahah tahrimiyyah) while his prayer remains valid on the soundest view. Al-Kasani's basis is that Allah commanded listening and keeping silent when the Qur'an is recited, and silence is possible even when the imam recites quietly, so it is obligatory by the text's apparent sense.
Evidence as quoted in the source (Qur'an, Arabic text verbatim):

Maliki school — Al-Fatihah is not obligatory on the follower; reciting is disliked in the audible prayer and recommended in the quiet one
For the Malikis, reciting al-Fatihah with movement of the tongue is an obligation on the imam and the solitary worshipper, not on the follower: it is not obligatory on him whether the prayer is audible or quiet; his reciting in the audible prayer is disliked even if he cannot hear the imam; the relied-upon view is that it is not required of him, and it is only recommended for him to recite it in the quiet prayer.
Evidence: Not stated in the passage consulted.
Internal difference within the school: Ibn al-'Arabi differed, holding it required of the follower in the quiet prayer; al-Dasuqi calls this weak, and the relied-upon view is against it.

Shafi'i school — Al-Fatihah is an obligation on the follower in every rak'ah, like the imam and the solitary worshipper
For the Shafi'is, reciting al-Fatihah, for one able to do so, is an obligation and a pillar of the prayer, specifically designated: neither its translation into another language nor any other Qur'anic recitation substitutes for it, and it is equally designated in all prayers — obligatory and voluntary, audible and quiet — for imam, follower and solitary worshipper alike. Whoever bows with the imam and then remembers that he forgot al-Fatihah, or doubts it, may not return to recite it, its place having passed and following the imam being obligatory; when the imam gives the salam, he must perform one rak'ah.

Attribution note: the detailed ruling on beginning with al-Fatihah and on one who forgets it appears in the matn of al-Shirazi's al-Muhadhdhab as reproduced and commented on in al-Majmu' — it is from the school's book and its commentary together.

Hanbali school — Obligatory on the follower in formulation, borne by the imam for him in performance
This position has two formulations in the school's own books, presented attributed as they appear: in al-Iqna' and its commentary Kashshaf al-Qina', reciting al-Fatihah is obligatory in every rak'ah on the imam, the solitary worshipper and likewise the follower, "but the imam bears it on his behalf" because of the report; in al-Muqni': "Recitation is not obligatory on the follower, and it is recommended that he recite in the imam's pauses, in what is not recited aloud, or where he cannot hear him due to distance"; and in al-Mughni, when the follower hears the imam's recitation, "recitation is not obligatory on him, nor is it recommended according to our imam." The practical effect is one when he can hear: the follower is not required to recite; only the juristic formulation differs between the books.
Evidence as quoted in the source (Hadith, Arabic text verbatim):

In simple words
For the Shafi'is, reciting al-Fatihah is an obligation on every worshipper in every rak'ah — imam, follower or solitary — in the audible and quiet prayers alike: the follower recites al-Fatihah behind his imam, and whoever forgets it and remembers after bowing with the imam does not return to it, but performs a rak'ah after the imam's salam. Opposite this, for the Hanafis the follower does not recite at all, and for the Malikis and Hanbalis the imam's recitation is borne on his behalf so it is not required of him.
The basis of the Shafi'i ruling, as in al-Nawawi's al-Majmu', is that al-Fatihah is a specifically designated pillar for which neither its translation nor any other Qur'an substitutes, per the hadith "There is no prayer for one who does not recite al-Fatihah," so it does not fall away from the follower, just as the other pillars do not.
Summary based on the Shafi'i position in al-Nawawi's al-Majmu', with the counterparts stated as information, not preference.

Does the imam pronounce the basmalah aloud in the audible prayer, or quietly?

Hanafi school — The basmalah is Qur'an but not part of al-Fatihah; it is recited quietly
For the Hanafis, the tasmiyah (basmalah) is Qur'an, but it is not part of al-Fatihah nor of the head of every surah; the worshipper recites it at the start of the recitation and conceals it, not pronouncing it aloud. Al-Kasani said: "It follows from this that the tasmiyah is not pronounced aloud in the prayer according to us, because there is no text for pronouncing it aloud, and it is not part of al-Fatihah such that it would be pronounced aloud by the necessity of pronouncing al-Fatihah aloud." And it is established from al-Hasan ibn Ziyad, from Abu Hanifah, that "he conceals Bism Allah al-Rahman al-Rahim at the start of the recitation, and need not repeat it in that prayer of his."
Evidence as quoted in the source (athar, Arabic text verbatim):

Maliki school — It is barred in the prescribed prayer and disliked there, and permitted in the voluntary prayer
Ibn Rushd said in Bidayat al-Mujtahid, verbatim: "Malik barred that in the prescribed prayer, whether audible or quiet — neither at the opening of the Mother of the Qur'an nor at the opening of the other surahs — and he permitted it in the voluntary prayer." In Khalil's matn and its commentary: the basmalah and the ta'awwudh are permitted in the voluntary prayer, in al-Fatihah and in the surah, and both are disliked in the obligatory prayer; al-Dasuqi said: that is, for the imam and others, quietly or aloud, in al-Fatihah or elsewhere, "and it was only disliked because it is not a verse of the Qur'an except in al-Naml."

Shafi'i school — The basmalah is a verse of al-Fatihah; beginning with it is required, and it is pronounced aloud in the audible prayer
For the Shafi'is, Bism Allah al-Rahman al-Rahim is a complete verse from the beginning of al-Fatihah, with no disagreement among them, and the worshipper must begin al-Fatihah with it; if he is in a prayer recited aloud, he pronounces it aloud just as he pronounces the rest of al-Fatihah aloud, and quietly where al-Fatihah is quiet.
Evidence as quoted in the source (Hadith, Arabic text verbatim):

Hanbali school — It is recited before al-Fatihah and concealed; pronouncing it aloud is not a Sunnah
For the Hanbalis, reciting Bism Allah al-Rahman al-Rahim is prescribed in the prayer at the beginning of al-Fatihah and of every surah, as in al-Mughni, and the Sunnah is for the worshipper to recite it before al-Fatihah and to conceal it. Ibn Taymiyyah said in Sharh al-'Umdah: the Sunnah is to recite the basmalah before al-Fatihah and to conceal it; and "the narration from Ahmad does not differ that pronouncing it aloud is not a Sunnah."
Evidence as quoted in the source (Hadith, Arabic text verbatim):

In simple words
For the Hanbalis, the worshipper recites Bism Allah al-Rahman al-Rahim before al-Fatihah and before every surah, but the Sunnah in their view is to conceal it and not pronounce it aloud even in an audible prayer: pronouncing the basmalah aloud is not a Sunnah for them. Opposite this, the Shafi'is pronounce it aloud in the audible prayer because for them it is a verse of al-Fatihah; the Hanafis likewise recite it quietly; and the Malikis hold that it is not recited in the obligatory prayer at all.
The basis of concealment for the Hanbalis is Anas's hadith: "I prayed with the Messenger of Allah ﷺ, Abu Bakr, 'Umar and 'Uthman, and I did not hear any of them recite Bism Allah al-Rahman al-Rahim" — so the Sunnah for them is to recite it while concealing it, as Ibn Taymiyyah set out in Sharh al-'Umdah and Ibn Qudamah in al-Mughni.

Is the qunut in Fajr prescribed permanently, or only for calamities?

Hanafi school — No qunut in Fajr; for them the qunut is in Witr
For the Hanafis, the qunut's place is the Witr throughout the year, before bowing, and there is no qunut for them in the Fajr prayer: because the Prophet ﷺ made qunut in Fajr for a month, supplicating against Ri'l and Dhakwan, then left it, so it was abrogated. Al-Kasani also argued that Abu 'Uthman al-Nahdi prayed behind Abu Bakr and behind 'Umar and saw neither of them make qunut in Fajr.
Evidence as quoted in the source (Hadith, Arabic text verbatim):

Maliki school — The qunut in the morning prayer is recommended permanently on the well-known view, quietly and before bowing
For the Malikis, the qunut is recommended in the morning prayer permanently — this is the well-known view (mashhur) — and what is sought in it is quietness, on the well-known view; its place is before bowing, as in Khalil's matn: "And a qunut, quietly, in the morning prayer only, and before bowing." Ibn Rushd said in al-Bidayah: "Malik held that the qunut in the morning prayer is recommended."
Evidence: Not stated in the passage consulted.
Internal difference within the school: Ibn Sahnun and al-Lakhmi said: a Sunnah; Yahya ibn 'Umar said: it is not prescribed; Ibn Ziyad has what indicates its obligatoriness; Ashhab said: whoever prostrates (for omitting it) has invalidated his prayer; and Ibn al-Fakihani said: the qunut for us is a virtue, with no disagreement I know of in the school on that.

Shafi'i school — The qunut in the morning prayer is a Sunnah without disagreement, after rising from bowing, and its omission is prostrated for
For the Shafi'is, the qunut in the morning prayer is a Sunnah with no disagreement among them, its place being after raising the head from the bowing of the second rak'ah; it is an emphasised Sunnah: if the worshipper omits it his prayer is not invalidated, but he prostrates for forgetfulness whether he omitted it deliberately or forgetfully. As for the other prescribed prayers, the sound, well-known view decisively held by the majority is that qunut is not made in them unless a calamity befalls the Muslims, in which case they make qunut in all of them.

Hanbali school — The qunut is not a Sunnah in the morning prayer except for a calamity, when the imam may make qunut
Ibn Qudamah said in al-Mughni, verbatim: "The qunut is not a Sunnah in the morning prayer, nor in any other prayer, except the Witr." If a calamity befalls the Muslims, the imam may make qunut in the morning prayer — Ahmad stated this expressly — "and individuals do not make qunut."

In simple words
For the Shafi'is, the qunut in the morning prayer is an established Sunnah which the worshipper makes in the second rak'ah after rising from bowing; it is an emphasised Sunnah whose omission does not invalidate the prayer, and the prostration for forgetfulness is made for it in their school. Opposite this, for the Hanafis and Hanbalis the qunut is not a Sunnah in Fajr — except, for the Hanbalis, for a calamity that befalls the Muslims — and for the Malikis it is recommended permanently as for the Shafi'is, but quietly and before bowing.
The basis of the Shafi'i ruling is Anas's hadith that the Prophet ﷺ made qunut for a month supplicating against tribes of the Arabs and then left supplicating against them, while as for the morning prayer, he never ceased making qunut until he departed this world, as al-Nawawi set out in al-Majmu'.

Does the worshipper raise his hands when bowing and rising from it, or only at the opening?

Hanafi school — Raising the hands is not a Sunnah in the obligatory prayers except at the opening takbir
Al-Kasani said: "As for raising the hands at the takbir, it is not a Sunnah in the obligatory prayers according to us except at the opening takbir." So he does not raise his hands when bowing or when raising his head from it; the reasoning is that this is a takbir performed in a state of transition, so raising the hands at it is not a Sunnah, like the takbir of prostration.
Evidence as quoted in the source (Hadith and athar, Arabic text verbatim):

Maliki school — On raising at bowing and rising from it there are two narrations from Malik: prohibition and raising
Al-Baji said in al-Muntaqa, verbatim: "As for the second place, it is at the descent into bowing and at rising from it: Ibn al-Qasim narrated from Malik its prohibition, and Ibn Wahb and Ashhab narrated from him the raising." The passage consulted does not expressly prefer either narration, so both are presented as they appear, without calling either of them "the well-known one." The school's companions supported Ibn al-Qasim's narration with 'Ali's hadith — "that he used to raise his hands at the start of the prayer and then not repeat it" — and al-Baji stated expressly that this hadith is, in his view, stopped (mawquf) at 'Ali; the basis of Ibn Wahb and Ashhab's narration is Ibn 'Umar's hadith above, which for al-Baji is, in his own words, "sound, agreed upon as to its soundness."

Shafi'i school — Raising the hands at bowing and at rising from it is a Sunnah in both
For the Shafi'is, raising the hands level with the shoulders is recommended for bowing, for rising from it, and at the opening takbir — for every worshipper: standing, sitting or lying, woman or boy, performing an obligatory or a voluntary prayer; al-Shafi'i stated it in al-Umm and the companions agreed on it. Al-Nawawi said: "As for raising them at the takbir of bowing and at rising from it, our school is that it is a Sunnah in both, and most of the scholars among the Companions, the Followers and those after them held it."
Evidence as quoted in the source (Hadith, Arabic text verbatim):

Hanbali school — He raises his hands at bowing and at rising from it, like his first raising
Ibn Qudamah said in al-Mughni, commenting on al-Khiraqi's words "and he raises his hands like his first raising": that is, he raises them to the level of his shoulders or to the lobes of his ears, as at the opening takbir, and the start of his raising is at the start of his takbir and its end at the takbir's end.
Evidence: Not stated in the passage consulted (Ibn 'Umar's hadith, the issue's basis, appears in the full al-Mughni passage beyond the excerpt presented here, so the evidence is recorded as not stated, per the verbatim-from-source rule).

In simple words
For the Shafi'is, it is recommended for the worshipper to raise his hands level with his shoulders at the opening takbir, at the takbir for bowing, and at rising from bowing — imam, follower or solitary, man or woman: raising in these two places is a Sunnah for them, like the opening. Opposite this, for the Hanafis raising is not a Sunnah in the obligatory prayers except at the opening; for the Malikis there are two narrations from Malik on the issue; and for the Hanbalis the raising is as for the Shafi'is.
The basis of the Shafi'i ruling is Ibn 'Umar's hadith that the Messenger of Allah ﷺ used to raise his hands level with his shoulders when he opened the prayer, when he made takbir for bowing, and when he raised his head from bowing he raised them likewise — narrated by al-Bukhari and Muslim, as al-Nawawi set out in al-Majmu'.
